The device is physical hardware, a handset or modem with a live SIM in it, sitting on the same network an ordinary phone uses.
Why it matters when you buy proxies
Mobile IP addresses are expensive for a site to block, because they're shared with real people. Carriers put large numbers of subscribers behind the same public address using carrier-grade NAT, so refusing one mobile IP also refuses everyone else sitting behind it. That collateral damage is the reason mobile addresses survive filters that flag a datacenter proxy on sight.
The trade-offs are cost and speed. Real hardware and a real data plan sit behind every mobile IP, so it costs more per address than any other type, and traffic crosses a cellular link, so latency is higher than a server in a data center. A concrete example
You're managing five client accounts on one platform and each needs to look like a separate person. Through a mobile proxy, each session leaves from a T-Mobile or Verizon address that the platform sees millions of times a day from genuine phones. Through a datacenter proxy, all five leave from a hosting company's address range that the platform can identify in a single lookup.
